HIPAA Privacy Policy

At Ascend Mind & Body, we are committed to protecting the privacy and confidentiality of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and related regulations. Patient Rights

  • Right to Access: You have the right to access and obtain a copy of your PHI held by us.
  • Right to Request Restrictions: You can request restrictions on certain uses and disclosures of your PHI assuming the restrictions do not interfere with the federal and/or legal requirements regarding PHI.
  • Right to Amend: You may request amendments to your PHI if you believe it is inaccurate or incomplete.
  • Right to Request Confidential Communications: You can request to receive communications of your PHI through alternative means or at alternative locations.
  • Right to File a Complaint: If you believe your privacy rights have been violated, you have the right to file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services.

Confidentiality and Security

We maintain administrative, physical, and technical safeguards to protect your PHI from unauthorized access, use, or disclosure. 

Changes to Privacy Policy

We reserve the right to revise this HIPAA Privacy Policy from time to time. Any updates will be posted on our website and effective immediately upon posting.
